I understand that neither the completion of this application nor any other part of my consideration for employment establishes any obligation for the company to hire me. If I am hired, I understand that either the company or I can terminate my employment at any time and for any reason, with or without cause and without prior notice. I understand that no representative of the company has the authority to make any assurance to the contrary.

PLEASE READ CAREFULLY BEFORE SIGNING

I understand that I may be required to submit to a urinalysis and/or blood test for the presence of drugs and/or alcohol as a condition of my employment. I also understand that I may be required to submit to a physical examination after an offer of employment has already been made in order to determine if I can perform the essential functions of the job. I agree to such an examination and/or testing at LMI Inc.'s expense. I authorize release of the results to LMI, Inc. and its use to evaluate my suitability of employment. I also release LMI, Inc. and its agents from any and all liability incident to the examination and/or testing. I also understand that if I am accepted for employment, I may be periodically required to submit to further physical examinations and/or urinalysis and/or blood tests for the same purposes and I agree to them on the same basis.

I further understand that I am not required to reveal to LMI, Inc. whether I have a disability or medical condition, or its nature or severity. LMI, Inc. is, however, entitled to ask whether I can perform the job-related functions of the position(s) I am applying for and may respond to my requests for a reasonable accommodation.
